This stuff does not seem to be easy to follow. I do pride myself on being quite bright and I'm missing straight forward 'this is how card sharing works and the differences between the different protocols'.

Am I missing it - does it exist?

Probably because 99% of users have no clue whatsoever, they rely on existing (closed) software and boilerplate guides.

You'll get a better idea by just browsing wikipedia:

In principle there is no difference between sharing protocols, the request is always an ECM extracted from the transport stream and the reply the decrypted code word(s) DCW that allows the mpeg audio/video parts of the transport stream to be csa-unscrambled for the next period (typically 10 seconds).
