I have the same problem. I have these:
DM600 with Gemini 4.60 from 19-12-2008
CCcam 2.0.11
CCcam 2.0.11/NewCS 1.65
TotalTV NDS3 card about year and a half old.
I don't have any external readers so I'm stuck with the internal. Didn't put the BoxID in CCcam.cfg neither, but will do it later or maybe tomorrow, and then I'll return with the result.

I managed to get TotalTV card working with DM600.
As Plonker said, one should just type boxkey in it's CCcam.cfg and this will work.
I tried both with CCcam 2.0.11 and CCcam 2.0.11/NewCS 1.65, and both EMU's work if there is BOXKEY typen in config file. I read somewhere on the web, that one should also specify voltage for internal card reader if local card sharing is wanted/option. Getting EMM's from many peers can get this cards warmed up over the maximum and then they melt. Unless you set operational voltage to 3,75MHz which will work fine with the card, and it will only get a little bit warm, but most important it will not melt.
This is what I have in mine CCcam.cfg to get this working:

You can get Your BOXKEY on Echolite receiver by going to menu, find Tool menu (Alatka), then Diagnostics (Dijagnostika), and look for Serial #. It will start with 0x... Your BOXKEY is all HEX-digits after 0x.
One thing more. I can't tell if mine card is new or old, but I have it for a little more then a year and a half. On NoviSF board I read that all receivers starting with 0x0B come along with the new types of cards (the ones which can melt) but I can't find any other site which can confirm this. So if somebody else knows more about this (what cards are new/old), please share it with all of us. Until, I'll assume that my card is new.
